The Service Request Form page is where retailers can go to report a problem with one of their machines.
Each service request is e-mailed out to the users specified in the Service Request Configuration page.
Information about the retailer location is automatically populated in the form when a user opens a request.
The following information is sent with each request.

Description
When the E-mail copy? option is checked, the user can have a copy of the service request sent to their own e-mail.
